How periodontal maintenance works

Unlike a standard preventative cleaning (prophylaxis), which is for patients with healthy gums, Periodontal Maintenance is for patients who have a history of periodontal disease.

Once you have undergone a “deep cleaning” (Scaling and Root Planing) to halt active infection, the bacteria that caused the disease are suppressed, but never fully eliminated. This specialized service is a deeper, more targeted cleaning at shorter intervals to disrupt bacterial colonies before they can re-attach to the root and cause further bone loss.

Who needs periodontal maintenance?

This service is the prescribed standard of care for anyone who has crossed the line from “gingivitis” (reversible) to “periodontitis” (chronic). It is the necessary pathway if:

  • Post-Deep Cleaning: You have recently completed Scaling and Root Planing therapy.
  • Deep Pockets: You have gum pockets measuring deeper than 4mm, which are difficult to clean with floss alone.
  • Bone Loss History: X-rays show you have lost some bone support around your teeth in the past.
  • Bleeding or Inflammation: You experience recurring bleeding despite good home care.
  • Implant Protection: Preventing “peri-implantitis” (infection around implants) in susceptible patients.

We use a specific sequence of treatments to keep the environment below your gum line hostile to bacteria but gentle on your body.

At every visit we update your periodontal chart, measuring the depth of the spaces between your teeth and gums to catch active disease early.

We remove plaque and tartar (calculus) from above and below the gum line, specifically targeting the root surfaces where bacteria hide.

Frequently asked questions

Why do I need to come every 3 to 4 months?

Studies show the specific bacteria that destroy bone repopulate and mature in periodontal pockets every 90 days (3 months). By cleaning them out at this interval, we “reset the clock” before they can cause damage.

Can I ever go back to a “regular” cleaning?

Generally, periodontitis is a chronic condition, similar to diabetes — once you have it, you manage it for life. While some stable patients may extend to 4 or 6 months, most remain on a maintenance schedule to prevent relapse.

Is it painful?

It should not be. If your gums are inflamed there may be slight tenderness, but we can easily manage this with local anesthesia. As your gums heal and tighten over time, the visits become very comfortable.

Will my gums grow back?

Gum tissue and bone lost to disease typically do not grow back on their own. The goal of Periodontal Maintenance is to freeze the destruction where it is and prevent any future loss.
